Capability Assessment for Diet and Activity Model Extended to Include Screening and Knowledge in Diabetic Adults

The hypothesis is that enhancing these capabilities, based on the Capability Approach framework, can significantly reduce hospital readmissions and improve diabetes management outcomes.
